Liability Release and Consent Form
 
Monavation x KŌVA Fitness Session
 
I hereby voluntarily waive, release, and discharge any and all claims, demands, or causes of action for personal injury, property damage, or any other loss that may occur as a result of my participation in a fitness session hosted by Monavation at KŌVA.
 
This release is intended to discharge in advance Monavation and KŌVA, including their owners, officials, officers, employees, contractors, volunteers, and agents, from any and all liability, including claims arising from perceived or actual negligence.
I acknowledge that participation in fitness and recreational activities involves inherent risks, including the possibility of injury or accident. With full knowledge of these risks, I voluntarily assume all responsibility for any injuries or damages that may occur. This waiver shall be binding upon my heirs, legal representatives, and assigns.
 
I hereby grant consent to participate in the above fitness session and affirm my acceptance of this liability release.
 
In the event of sudden illness or injury, I authorize emergency medical treatment by qualified medical personnel, including physicians or surgeons. I understand that neither Monavation nor KŌVA provides medical insurance coverage, and that any medical expenses incurred will be my sole responsibility.
 
I confirm that I have read, understood, and agreed to the terms of this Liability Release and Consent Form. I further affirm that I am of sound mind at the time of signing or completing this form electronically.
