LIVE VIRTUAL PERSONAL TRAINING INFORMED CONSENT, WAIVER, AND RELEASE

Participant Agreement: Personal Training Program

I willingly participate in a personal training program involving weight training and/or cardiovascular exercises, acknowledging the potential risks of injury.

Prior to engaging in any exercise activity or using exercise equipment, I affirm that I am physically and mentally fit, and it is advisable to obtain a doctor's examination.

I accept full responsibility for my dietary choices, including the use of supplements or products. Consultation with a physician is recommended before making any dietary changes.

I acknowledge that participation in both in-person and online live sessions involves inherent risks, including illness, injury, or even death, and I voluntarily assume these risks.

Monavation/Mona Fares and affiliated facilities are not liable for any loss of personal property during training sessions.

This waiver covers injuries resulting from the use of amenities, equipment, participation in activities, classes, programs, personal training, supervision, instructions, equipment malfunction, and slipping or falling on premises, including sidewalks and parking areas.
Release of Liability:

I have carefully read and fully understand this waiver, acknowledging it as a release of liability.

I voluntarily waive any claims or causes of action against Monavation/Mona Fares and their trainers/instructors for personal injury or property damage.

This release covers negligence claims and is applicable in all countries where allowed by law.

If any portion of this release is deemed invalid, the remainder will remain in effect.
Terms and Conditions:

Invoices unpaid by the 5th day after the due date will incur a non-removable $25 late fee.

Punctuality is essential. Sessions starting late will still end at the scheduled time. Being more than 5 minutes late without prior communication constitutes a no-show, with no makeup session offered.

This agreement operates on a month-to-month basis after the minimum 3 agreement, with no partial refunds. Account pauses or cancellations must be done before the billing date.

All workout routines, training materials, and program designs are the intellectual property of [Your Business Name] and may not be shared, reproduced, or distributed without permission.

No-shows without prior notice will not be rescheduled, and the session will be charged.

Client information, health records, and personal details will be kept confidential and used solely for the purpose of providing training services.
